New Patient Information

 

• New patients are required to fill out a new patient online questionnaire.

• Chiropractic questionnaires will be emailed to you to be completed online. Acupuncture questionnaires may be printed off and completed ahead of time or one will be provided for you to complete when you arrive at our office.

• Please arrive 15 minutes before your scheduled visit to allow time for paper work and Insurance verification.

What To Expect On Your First Visit

• Check in with our staff at the front desk.

• The doctor will bring you back to an exam room, take a brief history, and perform an exam to determine your treatment.

• After you have received your exam the doctor will perform your treatment and discuss if and when a return visit may be needed and why.

• If you have any questions during your visit feel free to ask, we are here to help you.
